An update on how to treat chronic GVHD

We realized recently that the regimen described for the administration of calcineurin inhibitor for treatment of chronic graft-versus-host disease (GVHD)1 is discrepant from current practice. Previously, we used an alternate-day regimen of calcineurin inhibitor administration,2,3 but we now administer cyclosporine or tacrolimus twice daily for treatment of chronic GVHD. Although results with the 2 regimens have not been formally compared, this change was based primarily on pharmacokinetic considerations.
