    Bone marrow (BM) biopsies from 21 MPL mutant (12 and 9 from MPLW515L and MPLW515K patients, respectively) were reviewed blinded concurrently with an equal number from JAK2wt or JAK2V617F mutant patients (n=42), randomly selected in our archive. Since it was preliminary determined that there was no statistically significant difference depending on the JAK2V617F mutational status of MPLwt patients, results from the two groups were combined. Three hemopathologists independently reviewed the slides (Visual Inspection), and the mean value from their reports was calculated. Total BM cellularity was corrected for patients’ age, the only patient characteristics disclosed to the hemopathologists; erythroid cellularity was categorized as “normal,” “increased,” or “decreased” and expressed as the percentage of patients showing either of these. Degree of reticulin was assessed on a 0 to 3 scale according to EUMNET criteria.2 For Quantitative Assessment, digital pictures of at least 10 different fields at 10× and 40× original magnification were taken from hematoxilin-eosin and Gomori stained slides using the Kodak Image Analysis Software, and they were processed using Adobe Photoshop 7.0 (Adobe Systems Inc, San Jose, CA). Digitalized images were subsequently reviewed blinded in order to annotate the following data: number of megakaryocytes (Mks) per section area (mm2), number of Mk clusters per section area, number of Mks comprising each cluster, percentage of small Mks over total number of Mks.3 Assessment of total BM cellularity and fibrosis was performed on digitalized images based on analysis of hystogram function according to http://rsb.info.nih.gov/nih-image/manual/windows.html# histogram; data were expressed as the percentage of section area occupied by cells or reticulin fibers (Gomori stain).§ χ2 test for trend.
