Protocol 2 was omitted in the original article. It consisted of phase 1 with daily dexamethasone for 3 weeks, weekly vincristine and doxorubicin ×4 starting at week 2, and 1-asparaginase ×4 during weeks 2 and 3, followed by phase 2 with cyclophosphamide on day 36, ARA-C 4×/wk plus intrathecal methotrexate on days 38 and 45.
