Copyright Transfer and Conflict of Interest Disclosure Form for Authors and Contributors to Blood

This form applies to manuscripts whose submission was begun in Blood Bench>Press on or prior to 5 p.m. (17:00) PDT Thursday March 27, 2008 (Friday March 28 at 12 midnight or 00:00 GMT). All manuscripts submitted after that date require an online signature of each coauthor using Blood Bench>Press. Consult the Author Guide for more details.

This form is composed of two parts, the Copyright Transfer Form and the Conflict of Interest Disclosure Form. To facilitate the process for the Authors, Blood requires only one set of signatures of ALL authors, at the bottom of this form. A signature certifies compliance with both parts of the form.

Manuscripts will not be processed until the Blood Editorial Office has received this signed form.

I. COPYRIGHT TRANSFER FORM

In order that the American Society of Hematology (ASH) may disseminate the articles or other contributions (“Works”) submitted and accepted to Blood, authors of such Works transfer to ASH the copyright of such Works.
If the Work has multiple authors, each author must individually transfer his or her right in the Work to ASH (Section A of this agreement) by signing this form.
If the authors received any funds from the National Institutes of Health (NIH) for the research reported in the Work, clause 2(i) of Section A of this agreement applies.
If the copyright in the Work is owned by the US Government, the senior or corresponding author or an authorized representative of the Government must sign Section B of this agreement.
If the Work is not published in Blood, Journal of The American Society of Hematology, this copyright transfer agreement will not take effect.

Section A

  1. The authors hereby transfer all copyrights in and to the manuscript named above (called the Work hereafter) in all forms and media, now or hereafter known, to the American Society of Hematology (ASH) effective if and when the Work is accepted for publication in Blood, Journal of The American Society of Hematology.

  2. The authors retain the following nonexclusive copyrights, to be exercised only after the Work has been published in final format in the print version of Blood.

  3. (a) Reprint the Work in print collections of the author’s own writing.
    (b) Present the Work orally in its entirety.
    (c) Use the Work in theses and/or dissertations.
    (d) Reproduce the Work for use in courses the author is teaching. (If the author is employed by an academic institution, that institution may also reproduce the Work for course teaching.)
    (e) Distribute photocopies of the Work to colleagues, but only for non-commercial purposes.
    (f) Reuse figures and tables created by the author in future works the author writes.
    (g) Post a copy of the Work on the author’s personal website, departmental website and/or the university's intranet, provided a hyperlink to the Work on the Blood website is included.
    (h)In all the instances under clause 2(a) through 2(g) above, the author will give proper credit to the original publication in Blood as follows:
    This research was originally published in Blood. Author(s). Title. Blood. Year;vol:pp-pp. © by the American Society of Hematology.
    (i) Effective September 2006, ASH and Blood are participating in the NIH PubMed Central (NIH Portfolio) Archive. ASH’s and Blood’s participation in this initiative means that Blood authors who published NIH-funded papers in the journal from May 2005 forward will not be obligated to submit manuscripts to the NIH archive because Blood will submit the final article on their behalf.
    (j) Effective October 2006, ASH and Blood have modified its access restriction policy to facilitate compliance with the Wellcome Trust requirement for authors to deposit papers in an open-access repository. For Wellcome-funded authors whose papers are accepted and publish in Blood, the journal requires an additional open access fee of $2000, in addition to the regular publication fees charged to authors. Upon payment of the fee, Blood will deposit the Wellcome Trust-funded article into PubMed Central and ensure immediate free access of the article on the Blood website and on PubMed Central.

  4. The authors understand that if and when the manuscript is accepted for publication in Blood, it will be prepublished online as a First Edition paper unless it is a Letter to the Editor, an Editorial, an Inside Blood item, or an Introduction to another article. The authors acknowledge that Blood's First Edition papers undergo full peer review but none of the editorial preparation, such as copyediting, typesetting, and proofreading, that is performed for articles published in the final print and online edition. It will be the corresponding author's responsibility to double-check the article for accuracy prior to submitting it for prepublication as a First Edition paper.

  5. The authors warrant that the manuscript (or its essential substance) has not been published other than as an abstract in any language or format and has not been submitted elsewhere for print or electronic publication consideration.

  6. The authors warrant that the manuscript does not contain any material the publication of which would violate any copyright or other personal or proprietary right of any person or entity.

  7. The authors will obtain and include with the manuscript written permission from any respective copyright owners for the use of any textual, illustrative, or tabular materials that have been previously published or are otherwise copyrighted and owned by third parties. The authors agree that it is their responsibility to pay any fees charged for permissions.

Section B -- For US Government employees only

  1. This section is to be used in the case of a "work made for hire", i.e., a work prepared by an employee within the scope of his or her employment or commissioned as a work for hire under a written agreement.

  2. The undersigned certifies that all or the majority of the authors or the primary author of the above manuscript are/is employed by the US Government and performed this work as part of their employment and that the manuscript is therefore not subject to US copyright protection. If there are coauthor(s) who are/were not US Government employees, the undersigned, as authorized agent, hereby transfers to ASH the copyright interests of such coauthor(s).

  3. The authors understand that if and when the manuscript is accepted for publication in Blood, it will be prepublished online as a First Edition paper unless it is a Letter to the Editor, an Editorial, an Inside Blood item, or an Introduction to another article. II. CONFLICT OF INTEREST DISCLOSURE FORM

ASH and the Blood Journal are committed to ensuring the integrity of all their activities. The Conflict of Interest Policy for Blood contributors requires each author to disclose all relevant financial and other interests that might be construed as resulting in an actual, potential, or apparent conflict in one’s role as contributor to Blood, regardless of amount or value.

Nothing in the disclosure statement shall be regarded as creating a presumption of impropriety in the existence of financial interests or other relationships. Blood’s purpose is to inform peer reviewers and readers of the existence of financial relationships pertinent to the article in the interest of the full transparency of the peer review and publication processes.

This form must be filled out and signed if and when any of the authors has any conflict of interest to disclose. It also needs to be filled out if none of the authors have a conflict of interest to declare.

During the peer review process the conflict(s) of interests will only be disclosed to editors and reviewers who will keep them confidential. If the paper is accepted for publication in Blood, all disclosures will appear in the prepublished article and in the final print and online edition of the article, under the acknowledgment section.
